Telehealth appointments may be Medicare subsidised if you have a valid referral from your GP under a Chronic Condition Management Plan (GP CCMP). If you are living with diabetes, you could be eligible for 5 Medicare subsidised appointments each year. You will need to ask your GP to write a referral letter and include your most recent pathology and medication list. Private appointments Alternatively, you can book a private appointment. You won鈥檛 be able to access Medicare rebates but may be able to claim rebates via your private health insurance. Talk to your medical insurance company to see if you are eligible. It is not possible to claim a Medicare rebate and a rebate from your private health fund at the same time. First diabetes appointment If it鈥檚 your first appointment, you need to book an initial consultation. This allows time to talk through your concerns, assess your condition and make a plan with your practitioner. You usually have a follow up review consultation 3-6 weeks later. This is to make sure your plan worked in the real world and so we can make any adjustments that are needed. What is a telehealth appointment? A telehealth appointment is just like a regular clinic visit but conducted via video call or over the phone. What do I need to do to prepare for a telehealth appointment? Why is the GP referred appointment cheaper? When a GP refers a patient to us, under a Chronic Condition Management Plan (GP CCMP) for an Allied Health appointment, the appointment is partially funded by Medicare. The standard fee is reduced by the Medicare payment and we only charge the patient the gap fee. What is an allied health professional? The defines allied health as 鈥渁 broad range of health professionals who can treat conditions and illnesses. Allied health care is provided by trained professionals with university qualifications. They use practices with good evidence of effectiveness to prevent, diagnose and treat various conditions and illnesses. They often work in teams with various other healthcare workers to provide specialised support to patients.鈥� When you are living with diabetes, relevant allied health services providers include health professions such as diabetes educator, dietitians, exercise physiologist, podiatrists and psychologists. People living with Diabetes, which qualifies as a chronic health condition, are entitled to a Medicare rebate on 5 allied health service appointments every calendar year. You access this via your GP using the Chronic disease allied health services referral form. Your GP will need to set up a GP Management Plan and a Team Care Arrangement.
